By completing this enrolment form, the named member is granted full membership and insurance cover. This applies to all participation, including any free taster classes, the 1‑month trial programme, and the full monthly training programme. Insurance cover is a legal requirement for anyone practicing martial arts.

 

I understand that the learning and practice of martial arts carries an inherent risk of physical injury. While Freestyle Martial Arts and its instructors will take all reasonable steps to minimise the likelihood of an accident, the risk of injury cannot be eliminated. Freestyle Martial Arts and its instructors accept no liability for injuries sustained during training.

 

​I agree to abide by the terms and conditions set out by Freestyle Martial Arts. I declare that all personal information provided is accurate and understand that I must notify my Chief Instructor of any changes. I acknowledge that false or outdated information may result in an insurance claim being rejected.

 

I also authorise Freestyle Martial Arts to take and publish photographs or videos of me/my child in class for educational and promotional purposes only.

 

By signing the enrolment form, I (or the parent/guardian of an under‑18 student) accept this declaration and the disclaimer of liability. I confirm that all relevant health conditions have been fully disclosed and understand that it is a legal requirement that I (or my child) possess the physical, mental, and emotional ability to safely participate in a martial arts programme.

ACCEPTANCE OF MEMBERSHIP:

Completion of Enrolment: Every new member must complete an online enrolment form before beginning any programme with Freestyle Martial Arts. By completing the enrolment form, the named member is granted full membership and insurance cover.

 

Membership Cost: Membership and insurance cover is included as part of the free taster class, 1‑month trial programme, and the full monthly training programme. There is no additional charge required.

 

Membership Approval: Acceptance of any membership application is at the sole discretion of Freestyle Martial Arts.

 

Verification of Information: Freestyle Martial Arts may verify any information provided. Supplying false or misleading information may result in the cancellation of all membership rights.

 

Binding Agreement: Acceptance of a membership application forms a legally binding agreement between Freestyle Martial Arts and the member (or the parent/guardian of a member under 18).

 

Terms & Conditions: All membership categories are subject to the terms and conditions in force at the time of the application.

 

Health Declaration: The member warrants that they are in good health and not knowingly incapable of participating in active or passive exercise. They further confirm that such exercise will not be detrimental to their health, safety, comfort, well‑being, or physical condition.

 

Insurance Requirement: Full membership includes public liability insurance, which is legally required in the practice of martial arts.

 

MEMBERS’ RIGHT TO CANCEL:

Right to Cancel: A member may cancel their membership at any time.

How to Cancel: Membership and insurance cover is automatically cancelled when you cancel your 1-month trial programme, or full monthly training programme. Cancellation can be made by notifying your Chief Instructor or Programme Director in person, by phone, or by email.

End of Membership: Membership remains active until the end of the month covered by the final monthly payment.

 

EXPULSION OR TERMINATION OF MEMBERSHIP:

Grounds for Immediate Termination: Freestyle Martial Arts may terminate the membership of any member, without notice and with immediate effect, if their conduct—whether or not a complaint has been made—is, in the reasonable opinion of Freestyle Martial Arts or its Instructors, injurious to the character, reputation, or interests of the organisation, or renders the member unfit to associate with others.

 

Loss of Rights: A member whose membership is terminated forfeits all membership privileges immediately and is not entitled to any refund of any programme fees.

 

THE CLUB FACILITIES:

Member Responsibility: Members are solely responsible for their use of any facilities provided by Freestyle Martial Arts.

 

Liability: Freestyle Martial Arts accepts no liability for indirect or consequential loss, damage, costs, expenses, theft, or damage to property, whether arising under contract or otherwise.

 

Additional Services: Certain services or facilities not included within specific membership types may be offered at an additional charge, at the discretion of the Chief Instructor.

 

Alterations to Facilities: Freestyle Martial Arts and its Instructors reserve the right to make reasonable changes to the facilities provided, without prior notice.

 

Operating Hours: Information on normal operating hours is available from the Chief Instructor. These hours may be extended or reduced at the Chief Instructor’s discretion, with or without prior notice, although reasonable notice will be given where possible.

 

Temporary Closures: When necessary, the club, or parts of it, may be closed for maintenance. Any refund due to such closure will be at the sole discretion of the Chief Instructor.

 

MONTHLY TUITION PAYMENTS:

Payment Responsibility: Monthly tuition payments are made directly to your Chief Instructor. Payments remain due regardless of irregular or non‑attendance until cancelled by the paying member in person, by phone, or by email. A minimum of 5 working days is required to cancel or amend a payment; otherwise, you may still be charged at your current monthly rate for the following month.

 

Payment Structure: Freestyle Martial Arts does not charge for the classes that are closed due to annual holiday. We offer 48 classes per year (excluding 4 annual holiday dates each year-one week at Easter and Summer, and two weeks at Christmas), split evenly over 12 months at the current monthly price, per person. All other classes are open and available for training.

 

Freezing Payments: If you will be absent for one month or longer due to illness, injury, work, or holiday commitments, your Chief Instructor may freeze or reduce your monthly payment until you return. Please speak with your Chief Instructor for details. Note that an absence of one month or more will extend the time required before your next grading.

 

CLUB RULES AND REGULATIONS:

  1. Students and spectators waiting to enter class must do so quietly and respectfully, with no running around. Please keep all corridors accessible so others can move freely about the building.

  2. Students must bow on entering and leaving class, bow to the Chief Instructor at the start of class and prior to dismissal, and bow to their partner at the start and end of each activity, or as directed by your Chief Instructor.

  3. Spectators are requested to keep all conversations to an absolute minimum, place phones on ‘silent’ and limit all movement in and out of class.

  4. All young children (non-members) must always be supervised by a parent or guardian and should remain seated during class for their own safety.

  5. Spectators and non-participating students must never enter or cut across the class floor while a lesson is active.

  6. Students must not run around, shout, ’play fight’ or use any equipment prior to the start of class.

  7. Students must gain permission from the Chief Instructor or Instructors to join and leave class.

  8. All students are asked to conduct themselves in a safe, respectful and mature manner while on the premises and at all times.

  9. The practice of martial arts must be undertaken safely, demonstrating courtesy, respect and integrity to others always.

  10. Parents or guardians are solely responsible for making sure children enter class safely and on time, and for the collection of their child, from class, at the end of the lesson. Therefore, we politely request that all parents or guardians collect their children on time and from either inside the class or just outside the class entrance and not in the car park or at the roadside. Freestyle Martial Arts and its Instructors are not responsible for the safety and welfare of its members outside of the classroom.

  11. Please notify your Chief Instructor at the start of the lesson if you are suffering from a new injury or illness that might be made worse through training, including coughs and colds. Your Chief Instructor will be able to advise you on the best course of action regarding your training.

  12. When arriving late, please report to either the Chief Instructor or Instructor who is responsible for student registration.

  13. All students are required to store their bags safely in an allocated ‘bag zone’ and not in the path of other students, visitors, entrances or exits. Coats, shoes and water bottles should be placed in or on top of your bag for additional safety.

  14. Students are not permitted to wear jewellery whilst training.  If jewellery is part of religious/cultural beliefs, permission to wear it while training must be gained from your Chief Instructor.

  15. Eating food, drinking alcohol or smoking is strictly prohibited anywhere on the premises.

  16. Any student training under the influence of alcohol or drugs will be severely disciplined.  Students taking prescribed medication that might affect performance should inform the Chief Instructor or Instructor prior to training.

  17. Swearing, spitting or chewing gum is not permitted in class at any time.

  18. In the extreme event of a fire, please listen carefully to, and follow the clear directions from, your Chief Instructor and Instructors who are fully trained to deal with such situations. For full fire safety regulations, please contact your Chief Instructor.

  19. At the start of class, students are required to line up immediately in height order when requested to do so by the Chief Instructor, with the smallest and youngest students at the front.

  20. All questions to your Chief Instructors or Instructors should finish with “Sir” (or Ma’am). E.g., “please can I have a drink, Sir?”

  21. All answers to your Chief Instructors or Instructors should finish with “Sir” (or Ma’am). E.g., “my best kick is a turning kick, Sir”.

  22. Strictly no talking when your Chief Instructor is addressing the class for safety and out of respect for the Chief Instructor and other students.

  23. Please keep conversations to a minimum during partner activities, unless in the case of sparring when no talking is expected.

  24. Students must gain permission from the Chief Instructor to join or leave the line-up if an unscheduled water or bathroom break is required.

  25. Please notify your Chief Instructor or nearest Instructor if you have suffered an injury or feel ill. If you feel sick, please go directly to the nearest bathroom- permission to leave is not required under these special circumstances.

  26. When the class is finished, students are requested to leave the class quickly and quietly, taking any conversations into the corridor so as not to disrupt the next class.

  27. Student mobile phones must be switched off (or on silent) throughout the class (unless the Chief Instructor has granted prior permission).

  28. A full safety kit must be worn during sparring. NO SPARRING KIT, NO SPARRING.

  29. Students must fully understand all rules and regulations and should seek guidance from the Chief Instructor if they do not. Failure to follow the above rules and regulations may result in disciplinary action and suspension or, in extreme cases, expulsion from the club. 

  30. Freestyle Martial Arts and its Instructors reserve the right to make judgement and decisions regarding the above rules and regulations and take any subsequent action deemed necessary.
